This site is intended for adults acting for a business or organization. By submitting company information, you represent that you are at least 18 years old, have authority to provide the information and will use the site only for lawful business purposes.
3. Savings estimator and illustrations
The calculator and examples are preliminary illustrations based on stated assumptions, including estimated net employer savings of $41.65 per enrolled employee each month, or approximately $500 annually, after a $50 monthly program fee. The employee example assumes a $1,198 monthly pre-tax election, a 22.5% illustrative tax rate and a $139 monthly management fee. These figures are not quotes, promises or guarantees.
Actual results depend on factors that may include employee eligibility, participation, compensation, tax rates, payroll configuration, program design, administrative costs and changes in law. A displayed estimate does not establish eligibility or create an obligation for Black Flag or a program administrator.
4. No professional advice
Site content is provided for general educational and informational purposes. Black Flag does not provide tax, legal, accounting, actuarial, medical or fiduciary advice through this site. 5. Program and coverage limitations
The preventative care management program described on this site is not health insurance and is not a qualified health plan. Services are subject to eligibility, enrollment, availability, provider terms and plan documents. Existing qualifying group health coverage may be required. Benefits and vendors may change.
6. No offer or client relationship
Using the calculator, submitting a form or speaking with Black Flag does not create an advisory, fiduciary, broker, agency or client relationship and does not constitute an offer of insurance, benefits, financing or tax services. Any engagement, program or service will be subject to separate review and written agreements.
7. Communications
When you submit a request and provide consent, Black Flag may contact you at the telephone number or email address you provide about the request and related services. Consent is not a condition of purchase. Message and data rates may apply. You may reply STOP to opt out of text messages or use the unsubscribe option in a marketing email.

You are responsible for ensuring submitted information is accurate, current and lawful. Do not submit employee-level records, Social Security numbers, protected health information, bank credentials or other sensitive personal information through this site. Benefits programs, payroll services, administrators and other third-party services are provided by independent parties under their own terms. Black Flag does not control and is not responsible for their acts, omissions, availability, performance, coverage decisions or data practices.
